Minnesota Department of Human Services Data Breach Investigation
The Minnesota Department of Human Services (“DHS”) and FEI Systems suffered a data breach affecting over 300,000 Minnesotans who use long-term care services and supports.
What Happened
In November 2024, FEI Systems, the vendor that manages the MnCHOICES assessment system, discovered that a provider-associated user had improperly accessed the demographic records of more than 300,000 people. For 1,206 of those individuals, additional personal information was also accessed. DHS began mailing notification letters to affected individuals in January 2025.
MnCHOICES is the system used by Minnesota counties, Tribal Nations, and managed care organizations to assess and plan services for Minnesotans who need long-term care and support services.
What Information Was Exposed
The breach may have exposed the following types of personal information:
- Demographic information (names, addresses, dates of birth, Social Security Numbers)
- Information related to long-term care services and supports
- Additional personal details for over 1,200 individuals
